Before getting into Hypertension ICD 10 guidelines, definitions of important keywords and examples of Hypertension ICD 10, first let us learn the definition of Hypertension and types of Hypertension

Hypertension:

Hypertension is a very common chronic problem that many patients especially the elderly ones suffer from. Accurate documentation regarding the cause/type of hypertension is the key to accurate coding of the diagnoses related to hypertension.

Persistent elevation of arterial blood pressure is diagnosed as hypertension.

Types of hypertension:

  1. Primary hypertension (Essential hypertension) – The most common form of hypertension with no identifiable cause. It accounts for 95% of people with hypertension.
  2. Secondary hypertension – Accounts for 5% of people with hypertension. Caused due to an underlying disease such as renal disorders such as chronic pyelonephritis, diabetic nephropathy etc.. and Vascular disorders such as coarctation of the aorta.
  3. White-coat hypertension – High blood pressure that occurs at the doctor’s office or in a medical setting, but not otherwise is called White-coat hypertension. Generalized anxiety is one of the causes of white-coat hypertension.
  4. Systolic hypertension– Refers to the elevated systolic blood pressure.
  5. Gestational hypertension– High blood pressure in pregnancy
  6. Hypertensive crisis- A severe increase in blood pressure that can lead to stroke.

Hypertension ICD 10 guidelines:

The below Hypertension guidelines have been taken from the ICD-10-CM Official Guidelines for Coding and Reporting FY 2021 provided by the Centers for Medicare and Medicaid Services (CMS) and the National Center for Health Statistics (NCHS).

  • Acute renal failure + chronic kidney disease– Both acute renal failure and chronic renal failure should be coded, and the sequencing should be based on the reason for the encounter.
  • Hypertensive cerebrovascular disease- Applicable code from I60-I69 is assigned first, trailed by the appropriate hypertension code.
  • Hypertensive retinopathy – Code from Subcategory H35.0 + code from category I10 – I15. Important Note: Sequencing should be based on the reason for the encounter.
  • Secondary hypertension is due to an underlying condition– Requires 2 codes- code for the underlying etiology + code from category I15. Important Note: Sequencing should be based on the reason for the encounter
  • Hypertension, transient– The code used is R03.0 (Elevated blood pressure reading without diagnosis of hypertension). In case of transient hypertension in pregnancy, the codes used are from categories O13 and O14.
  • Hypertension, Controlled– Refers to hypertension under control with treatment. Appropriate code from categories I10-I15 is assigned.
  • Hypertension, Uncontrolled– It denotes to untreated hypertension/hypertension not responding to present therapeutic regimen. Appropriate code from categories I10-I15 is assigned.
  • Hypertensive crisis– A code from category I16 is assigned. Any identified hypertensive disease from category I10-I15 is also coded. Important Note: Sequencing should be based on the reason for the encounter.
  • Pulmonary Hypertension– Coded using category I27. While coding secondary pulmonary hypertension any associated conditions or adverse effect of drugs or toxins is also coded.
